Parent Consent Form for FriendshipYOUTH

Friendship cautions our youth leaders from engaging in one-on-one communication / situations with students. However, we also recognize research indicates young people are more likely to keep their faith when connected to 5-7 strong adult relationships in the church. This form allows you to consent to certain mentoring relationships at your discretion. It can be revoked at any time. Please checkmark accordingly.

If not completed or any topic that is not checked, it is considered not allowed.

Friendship only allows generic messaging (announcements, scheduling, very brief ā€œhappy birthdayā€ texts, etc.) to be sent.

Friendship encourages group discussions and any one-on-ones to be done in public settings where others can view what is happening.

Friendship discourages providing transportation unless more than two people are in the vehicle. Friendship is not responsible for arrangements made by parents with others regarding carpooling to/from youth activities.

Mentoring/Discipling Guidelines

Friendship pastors, staff and youth volunteers make every effort to respect the privacy of our students. Should a youth worker become aware of life-threatening situations he/she will notify proper authorities, including the parents unless the parents are involved and the youth worker is worried of further harm to the student. Should the staff member/volunteer become aware of dangerous, but not life-threatening behavior/situations, the general procedure is to give the student a few days to bring the matter to their parent’s attention, and then the youth worker will follow up with the parents to ensure everyone is able to work through things together.

 I affirm that I am the parent/legal guardian of the applicant. I understand that to revoke any section of this form, I must contact the church to make any changes.
